HMG Announces Office Locations for 41 Physicians Leaving Novant Health System.

Holston Medical Group (HMG) will open 13 new office locations in the North Mecklenburg/Charlotte communities for the 41 physicians leaving Novant Health System effective March 19, 2019.

March 19, 2019 will be the first day the physicians will provide care as part of Holston Medical Group. Note that previous dates and locations released by Novant do not reflect any agreements between Holston Medical Group, the physicians and Novant.

Office locations have been selected as the new locations for the physicians transitioning to Holston Medical Group, view locations and providers here.

Patients who wish to continue care with their physician at their new locations are encouraged to contact Holston Medical Group by calling 1-877-HMG-1213 or by completing this form so that Holston Medical Group staff can contact them to establish a new account and schedule their appointments.

“The response from the community and from our patients has been overwhelmingly positive,” said Dr. David Cook, one of the physicians leading this transition. “In two weeks’ time, we have had nearly 5,000 patients complete the form online and indicate their desire to continue care with their physician. That’s what quality, compassionate care is all about; relationships. And we are fortunate to have excellent relationships with our patients that will carry forward and be supported by Holston Medical Group.”

Holston Medical Group will offer patients the convenience of online appointment scheduling, medication refill requests, secure messaging with the care team, and more through the online patient portal “MyHMG”. Holston Medical Group is known nationally for their electronic health records utilization and health information exchange, providing patients with more efficient, cost-effective care.

Contracts between Holston Medical Group and major insurance companies are expected to be finalized soon. Patients will continue to have access to their provider as contracts are worked out. All patients are welcome, and the physicians are dedicated to the continuation of care for patients.
