Do You Have a Doctor on Call?
To reach the doctor on call, dial 276-883-8000 and ask the operator to page the doctor on call.
Do You Have a No-Show Policy?
As of September 1, 2009, C-Health charges a $20 fee for missed appointments. Reducing no-shows helps us provide timely appointments for patients who need care.
What Is Your Refill Policy?
It is important that you do not run out of your medication. Please contact your pharmacy directly with refill requests, and they will send the request to our office.
Can I Bring Family Members With Me?
Family support is important. Patients are welcome to bring family members or caregivers to appointments when needed.
When Can I Be Seen?
Our goal is for patients to be seen on the day they would like an appointment whenever possible.
How Do I File a Complaint?
At C-Health, we strive to provide competent, compassionate, comprehensive, and committed care. If you have a complaint, please contact our Practice Manager, Terri Garrett.
How Do I Request Medical Records?
If you would like C-Health to release or obtain records from another healthcare provider, please complete a Medical Records Release Form and return it by fax or in person.
What Is the Best Process for Managing My Medications?
Please bring all prescription medication bottles to each appointment. This helps your provider review your medications, dosages, and refill needs accurately.
