The new 21st Century Cures Act requires healthcare providers to give patients immediate access to their protected health information (PHI) including clinic notes and test results. As a result, your provider may not have had an opportunity to review laboratory or other diagnostic results prior to them being made available to you.

How do I sign up?
HMG, C-Health & Seasons for Women patients
If you are an existing patient and would like to create a new FollowMyHealth
account, visit the portal, select Create New Account, and submit a
request to connect with Holston Medical Group.
Once your request is received, you will be sent an invitation with steps to complete your account setup.
Bristol Pediatric Associates patients
