What can I do with myHMG?
With myHMG, you’ll be on your way to better managing your life and your health. You may:
- Communicate with your provider/nurse team* (referral questions, test results, etc.)
- Request an appointment
- Request a medication refill
- View routine laboratory results
- Review medications, immunizations & allergies
- Review a summary of your office visit.

How do I sign up?
HMG & Seasons for Women Patients
If you are an existing patient and would like to create a new myHMG account, go to www.myhmg.com, click the Create New Account button, submit a request to connect to Holston Medical Group and we will send you an invitation to connect. When you receive the invitation, simply follow the steps provided.
To sign up, please visit any C-health location (Honaker, Lebanon or St. Paul). Once you have completed an authorization form at the location, we will send you information on how to access your account.
